Notice to Clients

The offices of the Animal Eye Care Clinics in Camarillo and Woodland Hills are permanently closed as of December 14, 2017. We are unable to provide your pet with medical care after that date. It has been our great pleasure providing for the ophthalmic care needs for many thousands of veterinary patients in the County of Ventura and surrounding counties since 1992, and we offer our very best wishes for you and your pets' future health!

At the beginning of November, 2017, the following information was emailed or mailed via USPS to all existing clients (those who have brought their pet to our office in the last 2 years). If you did not get this email, please check your spam folder. If we did not have a current email address on file for you, we mailed this information to you.

For some patients, this mail was returned to us because we did not have a current address on file. In case we missed you, here is the information again:

1. YOUR PET'S MEDICAL RECORDS -If you already have copies of your pet's Visit Summaries, you are all set and you do not need to ask for medical records. (At the conclusion of every visit to our clinic, you were provided with a Visit Summary, which provides the diagnosis and treatment plan for your pet.

You may make copies of these and give them to your new veterinary ophthalmologist - Your regular (referring) veterinarian was also provided with a copy of your pet's Visit Summary every time your pet visited our office. You may obtain copies of our Visit Summaries from your referring veterinarian, or have your referring veterinarian send them to the veterinary ophthalmologist of your choice.

Prior to December 14, 2017: - You may pick up a copy of your pet's Visit Summaries by visiting our office prior to Dec. 14, 2017 (kindly provide us with at least 72 hours notice prior to picking up your medical records, so that we can prepare them for you in advance) - We can transfer your pet's Visit Summaries to the veterinary ophthalmologist of your choice. Since these records are confidential, we will need your written authorization to make them available to another veterinarian

After December 14, 2017: - If you already have copies of your pet's Visit Summaries, you are all set and you do not need to ask for medical records.

Please note that there is a $25.00 charge for us to send your medical records after December 14, 2017.

To obtain medical records after December 14, 2017:

1. Please sign and return the medical records request form Download and either email us at medrecordsrequest@eyevet.com

OR

2. Sign and return the medical records request form Download to our mailing address: PO BOX 1228, 2150 Pickwick Drive, Camarillo, CA 93012

A customer service representative will then contact you to arrange for payment and confirm your email or physical mailing address.

** If you prefer that we mail copies of your Visit Summaries to you via USPS, please enclose a self-addressed stamped envelope - Requests for medical records will be processed within 5 days of our receipt.

Confidentiality -Your pet's medical records will remain confidential and on file for a date of three years from your last visit to our office, after which time they will be destroyed.

2. TRANSFERRING YOUR PET'S CARE TO ANOTHER VETERINARY OPHTHALMOLOGIST

Your regular veterinarian can recommend another ophthalmologist or visit the American College of Veterinary Ophthalmology (ACVO) website at www.ACVO.org and click on the "locate an ophthalmologist" button.

3. PRESCRIPTION REFILLS - Any existing prescriptions for your pet that have been filled at a pharmacy will remain valid, until that prescription expires.

- Once the prescription expires, you will need to have your referring (regular) veterinarian or your new veterinary ophthalmologist give you a new prescription.

- By law, your veterinarian or veterinary ophthalmologist will not be able to refill any prescriptions without first examining your pet.
